We collect the following non-public personal information: (i) information we receive from you, including, your title, name, address, email address, phone number, and fax number; (ii) information material to why you or one of your family members may be interested in services or information provided by Cornerstone and/or psychologists providing services at or through the Cornerstone clinic, other health professionals, directors, officers, employees or consultants of Cornerstone (the “Cornerstone Personnel”); (iii) the web server automatically records your IP address when you visit the Website. This information may be used in aggregate form to help us understand how visitors are using the Website, for server and network operation and maintenance, to help us understand general usage patterns, and in circumstances where we are concerned about unauthorised use of the Website; (iv) the Website may use cookie and tracking software to personalize and enhance your visit to the Website. Cookies are small text files that are placed on your computer by the websites that you visit. Personal information cannot be collected via cookies and other tracking technology; however, if you previously provided personally identifiable information, cookies may be tied to such information.

The Way We Use Information

We use the information you provide about yourself, your child, or your family: (i) when assessing whether Cornerstone’s services may be appropriate for you or your family; (ii) for referring you to one of the Cornerstone psychologists or other health care practitioners; (iii) for communicating with you regarding the services or information that you request and to respond to your requests and inquiries; (iv) to manage and administer our business.

If we deem it appropriate to refer you or a family member to a psychologist outside of the Cornerstone clinic, we will always obtain your permission first. We do not share the personal information that you provide to us with parties external to Cornerstone except with your express written consent other than in the following circumstances: (i) where required by law or to comply with codes of professional conduct; (ii) where we have a reasonable basis to believe that you are in breach of the terms and conditions of our Website; (iii) for the purpose of participating in actual or threatened  legal proceedings or alternative dispute proceedings initiated against Cornerstone or any of it officers, directors, principals, employees, contractors or affiliates.

In addition, in aggregate form, information relating to users of the Website may be provided to third party advertisers and business partners for editorial feedback purposes, marketing and promotional purposes, statistical analysis of user’s behavior, product development, content layout and improvement. No personally identifiable information in provided.

Data Security

Cornerstone has implemented reasonable physical, organizational and technological security measures to safeguard personal information in our custody or control. However, given that electronically submitted data is not 100 percent secure, we make no warranties as to the security of any such personal information that you submit and you do so at your own risk.

How You Can Access or Correct Your Information

You are entitled to request access to and correct all your personal data collected by us. You are also entitled to require us to delete your personal data if we have not yet, and you no longer want us to process it. Cornerstone may however retain your personal data for specific time periods to comply with professional obligations.

Assignment

Cornerstone reserves the right to transfer all data in its possession to a successor-in-interest to its business or assets.

Cornerstone controls and operates the Website from in and around Vancouver, British Columbia, Canada.
